SEOUL (AFP) – One of five wild ducks found dead in South Korea this week was confirmed Saturday to have been infected with a lethal strain of the bird flu virus as the country battles its first outbreak in over two years.

Tests showed one of the five dead birds found in Sacheon City on December 26 had been stricken with the H5N1 virus, the agriculture ministry said in a statement.

The same strain -- which poses a risk to humans -- has also been detected in wild birds and their faeces at four other locations across the country since December 7, it said.

　　 Article 1 These Implementation Rules are formulated in accordance with the Frontier Health and Quarantine Law of the People's Republic of China (hereinafter referred to as "the Frontier Health and Quarantine Law). Article 2 For the purpose of the Frontier Health and Quarantine Law and these Implementation Rules, the definitions of the following terms are:"Inspection" means that the Frontier Health and Quarantine Organ (hereinafter referred to as the "Health and Quarantine Organ") carries out medical inspection and sanitary inspection. "Quarantinable Epidemic Victim" means a person who suffers from quarantinable infectious disease or a person who has been proved through primary diagnosis by the Health and Quarantine organ to have caught the quarantinable infectious disease or have become an incubation carrier. "Quarantinable Epidemic Suspect" means a person who has been exposed to the quarantinable epidemic environment and may transmit quarantinable infectious diseases. "Isolation" means that a person affected by a quarantinable epidemic disease is being detained in a designated place for medical treatment until there is no longer any risk of spreading the disease. "Check-up detention" means that a suspect carrier of a quarantinable epidemic disease is being detained in a designated place for diagnosis and check-up. "On-site clinical check-up" means that a person is required to go to the nearest health and quarantine organ or medical treatment unit for diagnosis and check-up within a specific period of time set by the health and quarantine organ or that the person is visited by people either from the health and quarantine organ or from the medical treatment unit for diagnosis and check-up. "Transportation facility" means cargo containers. "Sanitization" means both medical measures, including isolation, check-up detention and on-site clinical check-up; and "sanitary measures" including disinfection, deratization, and deinsectization. "To monitor epidemic diseases" means to carry out survey in a specific environment and among specific groups of people on matters relating to epidemiology, serology, etiology, and clinical symptoms; and to make prognostication of the incidence, development and spread of an epidemic disease. "Health supervision" means the sanitary inspection, identification, evaluation and sampling conducted in the implementation of the health law and hygienic standard. "Conveyance" means vessels, aircraft, trains and other motor vehicles. "Frontier ports" means international seaports, airports, railway or bus terminals on the border, and ports of entry at land frontiers and boundary rivers. Article 3 Duty range for health and quarantine organs on the frontier shall cover those hotels, restaurants and clubs on the frontier and those units that provide food or services for the incoming or outgoing conveyance and those places where quarantine inspection, sanitary monitoring and health supervision on the incoming or outgoing persons, conveyance, containers and goods are carried out. Article 4 All persons, conveyance and containers, as well as articles such as baggage, goods, postal parcels that may transmit quarantinable diseases are subject to quarantine inspection upon entering or exiting the country in accordance with these Rules, and entry and exit shall be allowed to them only after an approval is issued by the health and quarantine office. A 22-year-old male from the northern province of Bac Kan, the sixth bird flu case since the beginning of the year, is in serious condition and requires the use of a breathing machine, said the hospital’s director Nong Quoc Chi.

The patient was transferred to the hospital from the province’s General Hospital on April 3 with symptoms including a cough and high fever.

Tests conducted by the National Epidemiology Institute on April 4 showed he was positive for the bird flu virus (A/H5N1).

An investigation has revealed there were dead waterfowl in the man’s neighborhood. Four other residents from the man’s commune also showing bird flu symptoms have been quarantined and are also under treatment.

Intranasal SARS vaccine protects monkeys

Jun 25, 2004 (CIDRAP News) – One dose of an experimental intranasal vaccine protected monkeys from the SARS (severe acute respiratory syndrome) virus in a study reported this week, raising hope for a vaccine that could quickly protect healthcare workers in a SARS outbreak.

African green monkeys that received the spray vaccine showed no viral replication after they were exposed to the SARS virus, whereas the virus reproduced in unvaccinated monkeys, according to the report in The Lancet. The study was conducted by Alexander Bukreyev, PhD, Peter Collins, PhD, and colleagues at the National Institute of Allergy and Infectious Diseases (NIAID).

"This study shows that delivering the vaccine directly into the respiratory tract can effectively protect primates from SARS," said Brian R. Murphy, MD, a coauthor of the study, in an NIAID news release. Murphy is co-chief of the NIAID Laboratory of Infectious Diseases.

"With more research, we hope to develop a vaccine based on this approach that could be used to rapidly immunize first responders and other medical personnel, helping them control a potential outbreak," Murphy added.

The researchers created the vaccine by combining a piece of the SARS virus with a weakened version of human parainfluenza virus type 3 (HPIV3), which in its natural form causes respiratory diseases in young children, the NIAID said. The researchers altered the weakened virus, called BHPIV3, by adding a gene for the SARS spike protein, which protrudes from the surface of the SARS virus and enables it to attach to human cells. Using BHPIV3 made it possible to introduce the SARS spike protein directly into the respiratory tract, the NIAID said.

Four monkeys received a nasal spray containing BHPIV3 with the SARS spike protein gene (BHPIV3/SARS-S), while four other monkeys received a version of BHPIV3 containing a harmless gene in place of the SARS gene, according to the NIAID. The monkeys that received the test vaccine developed neutralizing antibodies against the SARS virus, while the control group did not.

The concerns regarding D225G and D225N began to increase whensequences were released from fatal cases in Sao Paulo Brazil. These samples were collected in July, when the flu season in the southern hemisphere was peaking. Two lung samples were positive for D225G, while a lung and throat sample were positive for D225N.

There already was interest in position 225 for a number of reasons. D225N was in seasonal H3N2 and linked to the fixing of adamantine resistance (S31N), while D225G was in 1918 and 1919 samples and linked to a change in receptor binding domain specificities which would target subsets of cells in the lung. In addition, the polymorphism was jumping from one genetic background to another signaling recombination and increased chances that the polymorphisms would continue to jump to new genetic backgrounds.

The Refik Saydam National Public Health Agency has released 29 HA sequences from Ankara, Turkey. Several were partial sequences, but 26 covered the receptor binding domain and 8 had D225E (see list below), while one had D225N, A/Ankara/26/2009.

The outcomes of these patients were not given but media reports have described a rapid in increase in H1N1 fatalities in Turkey.

A large number of HA sequences with D225E has been published from Spain, and recently released GISAID sequences from Sweden and the UK also haveD225E, including three fatal cases from Sweden.

The prior reports of D225G and D225N associations with fatal cases has raised concerns that changes at position 225 could alter tissue tropism or aid in immune escape, leading to more severe and fatal cases.

The central province of Quang Binh and the northern province of Dien Bien have strengthened measures to cope with A/H1N1 flu while the deadly virus is expected to spread widely during this winter.

In Quang Binh, more than 500 people took part in a rehearsal to prevent A/H1N1 flu infections in humans in Ba Don town, Quang Trach district. Health workers from the provincial district preventive medical centres took turns checking suspected patients, isolating them and taking samples for test at the central hospitals.

Meanwhile, in Dien Bien, four more cases of A/H1N1 flu infection have been reported, bringing the total cases in the province to 19. So far, the disease has spread to 30 communes and 37 schools in the three districts of Dien Bien, Muong Cha and Dien Bien Phu.

According to the provincial preventive health centre, since the first H5N1 patient died in Dam Mun, Dien Bien district on November 30, 2009, no more deaths from the same disease have been reported in the province. Previously, four suspected cases tested negative for the A/H5N1 virus. Currently, 18 people contacting the deadly virus are being isolated and monitored strictly and they remain in stable conditions.
